Cancer Care Coordinator

Cancer care coordination is available to all patients diagnosed with cancer and provides guidance to assist patients and families in navigating their cancer care. The focus on the program is to eliminate barriers to care by offering support and providing resources for the difficulties that one faces when dealing with cancer, psychosocial, emotional, and financial. The Wilmington VAMC Cancer Care Coordinator is Mary Ann Graham, RN, BSN, CHPN.

We can help you:

  • Make this time less difficult
  • Learn about clinical trials for cancer
  • Find resources for medical coverage, medications, transportation, and other important services
  • Connect with support groups and counselors
  • Get information about your disease and symptoms
  • Find out about nutritional programs and services that can keep you strong
  • Arrange for home care services
  • Work with your insurance company
  • Locate financial assistance
  • Connect with government services such as the Delaware Cancer Treatment Program (1-800-996-9969), Screening for Life (1-800-464-HELP) or other government programs that may be available to you.
